Google Forms is the often overlooked quiet child of the Google family.  Don't let that fool you, this is just as great as all of the other GAFE tools.  Google Forms can be used with even the youngest students (independently or with a little help) for all sorts of uses.  Forms can be used for formative assessments, pre-assessments, to review concepts, survey students, promote engagement, and create competition.  One of the best things about Forms is the ease of use. Just like all Google Apps for Education, you can collaborate on the form itself or share results.   It works on all devices and students can access the form by scanning a QR Code, or clicking on (or typing in) a link.  Start by setting up the questions, share and encourage completion, and it automatically compiles the results for you.  Results are now shown with graphs and other helpful visual representations, or can be viewed by individual responses and best of all a Sheet of responses is automatically created as well.  Learn more about how easy this is and start using Google Forms.

FAQ

Do you need to have (or sign in to) a Google Account to RESPOND to a Google Form?

No.  You can set up your form to allow anyone to complete the form, but if you do it is STRONGLY suggested that you add a name field (unless you want it to be anonymous).  However, it can also be restricted to only those with MISD accounts, or designed to automatically collect user IDs from those who respond.  If either of these is part of the design, then signing in WILL be required to complete the form.

 

How can I share the form with students/parents/co-workers to complete?

When you are ready to send the form a link will automatically be generated.  You can send this link out in email or post it online.  

How can I set up a Quiz on Google Forms that will grade itself?

Use Google Forms Quizzes and grading has never been easier.  Check out this tutorial to learn more, and then challenge yourself:  How could you use quizzes to differentiate?  Mr. McDaniel explains how this works in his video tutorial.  Simply use feedback!  When you get it wrong you need to watch an extra video or examine an extra resource, but this is not required if you get the answer correct.

 

What other Ad-Ons can I use?

There are many great ad-ons available for forms.  This resource by Ctrl+Alt+Achieve outlines 9 awesome ad-ons teachers should consider using, including my favorite: Certify'em.  

 

How can I use Google Forms with student projects?

Think about using Google forms for check-ins, formative and summative assessment as well as student surveys before during an after units.  Student surveys can be used to find out student preferences, strengths, weaknesses as well as give you some feedback.  Also think about how to use Branching as a project check-in tool.

Are Google Forms only for the "Big" kids?

No!  Google Forms and Quizzes can be used with even the youngest students.  The easiest way to share with young learners is probably through QR codes or See-Saw but forms can be used with audio, video and images making them accessible for young students, see this example to learn more.  Consider how easy your CFAs could be with Google Forms!

Challenge

Task 1:  Learn about Forms using the resources above.  Then start by creating a form for use with students and/or parents.  Use differentiation (branching) at least once.  Prepare for how you will share this as well.

 

Task 2:  Uses for Google forms are unlimited.  Be creative, use your own imagination or search for ideas online.  Create a form that goes beyond the basic survey or quiz for use with your students.

 

Task 3:  Create a short pre-quiz or quiz for your students and set Google Forms up to grade it.
